If an employee or any insured dependent uses a tobacco product, a $40 per pay period surcharge will be added. If an employee's spouse is eligible for other medical insurance through their employer or Medicare, they will be excluded from the City of Spartanburg Medical Plan.

 

Dental Benefits

Orthodontic Services  50% of allowable charges up to a maximum of $1,000 per member, per lifetime.

Maximum benefit: $2,000 per person in a benefit year

Benefit Waiting Period: First day of the month following the date of employment. Additionally, there is a 12 month waiting period for major services and orthodontic treatment.

* Preventive services include routine examinations, cleanings, and diagnostic X-rays.

* Basic services include fillings, endodontics, periodontics, and oral surgery

* Major services include crowns, bridges, and dentures.
